Throughout this era of rapid change and disruption, we have been the conduit for training and educational experiences that truly support the mission-based provider. This presentation describes the proposed Patient Driven Payment Model (PDPM) that will be implemented in SNFs starting October 1, 2019 along with the resulting care management issues. We will also share the next steps that should be taken to prepare for the upcoming reimbursement changes.

CONTINUING EDUCATION
NHA: LeadingAge Michigan Educational Foundation has applied for continuing education contact hours with the Michigan Board of Nursing Home Administrators.

NURSES: LeadingAge Michigan Educational Foundation (WNA-0558-001) is an approved provider of continuing education by the Wisconsin Nurses Association, as accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

In order to receive the continuing education credit for the webinar you must, sign in to GoToMeeting, stay for the entire length of the webinar and complete the evaluation at the conclustion of the program. Once the evaluation has been completed you will be emailed your certificate of attendance. Certificates can also be printed upon course and evaluation completion by logging into your profile at www.leadingagemi.org


One (1.0) contact hour will be awarded per successful completion.

  • Completion of online evaluation
