Readmissions Matter (Now More than Ever)

For the past decade, we have watched an increasing focus on readmission rates – by payers and insurers, hospital administrators, and policymakers. About 20% of hospital discharges to skilled nursing facilities continue to be readmitted within 30 days, and by 90 days you can expect about 33% of patients will have made it back to the hospital. The cost to Medicare for these readmissions is in the billions. Additionally, readmissions are often considered a proxy for quality of care:

  • Readmission rates are a marker for harm to the patient
  • Readmissions decrease the patient experience with their overall care
  • Readmissions cause a huge emotional toll for patients and families
  • A large portion of readmissions are considered wasted revenue and inefficient use of resources
  • About 2/3 of readmissions are considered avoidable

For skilled nursing providers, these readmission rates are more important than ever. Two percent of traditional Medicare provider payments are at risk. CMS will be initiating its SNF Value Based Purchasing Readmission Measure – called the SNFRM – for facility performance during calendar year 2017. Performance for that measure will inform member rates for fiscal year 2019, which starts October 1, 2018.

Working from baseline data established during the 2015 calendar year, CMS will be measuring overall performance based on readmissions and improvement from the baseline every year, starting with performance during calendar year 2017. On the surface, the program seems to be designed to simply cut Medicare SNF rates since 40% of the withhold will be retained by the federal government. Only 60% will be distributed to facilities based on performance.

The SNFRM uses both an achievement score which compares a member facility performance rate during a performance period (initially CY 2017) against all SNF performance during the baseline period (2015). Improvement scoring compares SNF performance during the performance period against its own prior performance during the baseline.

Every year, CMS will define certain parameters for scoring. For achievement, the threshold will be set at the 25th percentile of performance. Using the baseline year of 2015, the threshold is 20.41% a risk adjusted readmission rate - - - note that CMS converts this to a 79.59% threshold. Providers who do not meet this basic threshold (the minimum level of expected performance) will receive 0 points for Achievement.

CMS has identified its benchmark (goal for 2017) as the mean performance within the top decile of performers or for 2015, a 16.4% readmission rate (again converted to 83.6%). Facilities who achieve this benchmark will receive all 100 points. CMS will provide points for those facilities that score between the threshold and the benchmark based on a complicated formula.

The improvement score is determined by comparing the facility’s improvement from the baseline period. Between 1 and 89 points will be awarded. CMS will award each facility the higher number of points received between Achievement and Improvement. The scores are not added together. CMS will issue performance data in preview reports that should be provided about two months before the beginning of the 2019 fiscal year.

The Protecting Access to Medicare Act of 2014 (PAMA) focuses on potentially preventable readmissions. The SNF-PPR will measure a risk-standardized rate of unplanned readmissions for Medicare FFS SNF patients within 30 days of discharge from the hospital. Eventually, this measure will replace the SNFRM noted above.

Successful member facilities will have in place a robust root cause analysis process for every unplanned readmission as well as a comprehensive care management program that actively engages all of the interdisciplinary team. A genuine plan of care that is person-centered will go a long way to addressing many of the issues that ultimately lead to readmissions.

New Quality and Certification Oversight Reports (QCOR) website

The Centers for Medicare & Medicaid Services (CMS) have launched a new website to share survey and certification data for a variety of federally-certified provider types, including nursing facilities, hospice and home health agencies.

The Quality and Certification Oversight Reports (QCOR) website replaces the Survey and Certification Providing Data Quickly (S&C PDQ) system, and allows users to run reports that can be customized by designating provider characteristics, survey type and other factors. CMS updates the data on a weekly basis.

The website offers these report options:

  • Provider Reports: Active Provider and Supplier Counts, New Provider and Supplier Counts, Terminated Provider Counts
  • Survey Reports: Overdue Recertification Surveys, Recertification Survey Counts, Survey Activity Report
  • Deficiency Reports: Deficiency Count, Average Number of Deficiencies, Citation Frequency, Double G Citations Report
  • Enforcement Reports: Enforcement Actions, Civil Money Penalty (CMP)
  • Abuse Reports: Abuse Citation Rates

In announcing the new site and data platform, CMS said: “S&C PDQ provided summarized survey and certification data, including results of on-site inspections of providers and suppliers. The system imports data from a variety of data platforms, such as the Certification and Surveyor Provider Enhanced Reporting (CASPER) national database. However, S&C PDQ’s functionality and ease of access of information to providers and suppliers was limited, therefore CMS removed the requirements for username and passwords for accessing the data on the site, which has now become the new QCOR website.”

Senior Care Resources – Be Part of a High Performing Network

Senior Care Resources (SCR) members continue to outperform all other nursing home providers in Michigan. According to the most recent quality metrics available, SCR members have an average 5-Star rating of 4.9 compared to the Michigan Nursing Home average of 3.5. In addition, SCR members average 3.8 Stars for Inspections (Survey) which outperforms the state average of 2.7; a 4.4 Staffing rating compared to a state average of 3.5 a 4.8 QM rating compared to 4.0 for the state; and a 4.5 RN rating compared to 3.7 for all state facilities. SCR members are to be commended for their consistently high level of performance compared to other providers in the state.

Getting Ahead of Sepsis – CDC

For many hospitals, including those in Michigan, sepsis is the leading primary diagnosis for seniors covered under Medicare. Sepsis is a life-threatening condition characterized by a ‘whole body’ response to infection. Timeliness of treatment is crucial, as sepsis can lead progressively to tissue damage, organ failure and death. Sepsis is also a leading condition provoking hospital readmissions.

The Centers for Disease Control has just announced its ‘Get Ahead of Sepsis’ Effort with an array of education materials. Members can view the program details at

SNF Placement – It is All about Location

Last week, the Medicare Payment Advisory Commission (MedPAC), issued a brief on use of higher quality post-acute care providers. MedPAC is examining the role of hospitals in discharge planning and the factors that influence the beneficiary decision-making process. Almost half of skilled nursing facility residents had five or more higher quality facilities available in their area that they were not directed to by a referring hospital, according to the study.

About 15% of residents were discharged to the facility with the highest quality rating within a 15-mile radius; almost half went to facilities that demonstrated lower quality ratings by five or more providers within that radius. These differences in quality were identified as ‘non-trivial’ by the panel.

MedPAC is recommending modifying discharge planning rules to allow hospitals to recommend specific nursing home and home health providers along with the possibility of discharge planners to openly discuss provider quality ratings.

MI Choice Stakeholder Meetings Announced

The Michigan Department of Health and Human Services (MDHHS), Medical Services Administration issued numbered letter L 17-30 announcing stakeholder meeting dates associated with the renewal process for the MI Choice home and community based services waiver program. MDHHS is required to solicit input from stakeholders regarding the waiver program as part of the renewal process with the Centers for Medicare and Medicaid Services.

The initial stakeholder meeting will be held on September 28, 2017 from 1:00 to 4:00 PM in the Capitol Commons Center located at 400 S. Pine Street in Lansing. Additional information regarding the stakeholder meetings, including future dates, is outlined in the letter linked above.

Jimmo Settlement Information

CMS has announced the launch of its ‘Jimmo Settlement’ website that was required as a result of the 2013 Settlement Agreement. The Jimmo settlement required a clarification of Medicare policy for providing therapy to individuals to maintain a person’s condition or slow the deterioration of a condition. The CMS webpage offers resources and responses to frequently asked questions that may assist providers in understanding the conditions in which skilled nursing and therapy can be provided and billed.

Important Proposed Medicaid Policies Issued

The Michigan Department of Health and Human Services (MDHHS) Medical Services Administration has issued four proposed Medicaid policy changes that will impact providers in the near future. The policies affected are 1) Asset Verification Program; 2) Modernizing Continuum of Care – Changes to Eligibility Inquiry/Response Transactions and Unique Health Plan Identifier; 3) Elimination of the Paper Version of the Facility Admission Notice; and 4) Modernizing Continuum of Care Project. Clearing Up Confusion on the SNF Quality Reporting Program

The Skilled Nursing Facility (SNF) Quality Reporting Program has experienced many implementation issues since it began October 1, 2016 leaving care centers struggling to make sense of how to comply.

There has been much communication from the Centers for Medicare & Medicaid Services on issues, corrections, changes to reporting periods and implementation of the 2% payment reduction for “non-compliant” providers as well as changes to the Quality Reporting Program for the next two fiscal years.
To help care centers better understand the dates, deadlines, submissions and reports, LeadingAge had provided a summary of the following:

  • Effect of Medicare Fee-for-Service Payments beginning Oct. 1, 2017
  • Technical Coding Issues Identified for 2017 Data Delays Submission Deadline
  • Data Collection and Submission Deadline tables for MDS data in 2018, as well as the impact on FY2018 and FY2020 Medicare Fee-for-Service rates.

Influenza and Pneumococcal Immunizations

Federal tag F883 requires nursing homes to inform residents of the benefits and risks of the influenza and pneumococcal immunizations and ensure that each resident has an opportunity to receive them unless they are medically contraindicated, or they have been refused by the resident or perhaps have already been immunized. To avoid a citation for noncompliance with F883, which has the potential to result in substandard quality of care, the Centers for Medicare and Medicaid Services (CMS) provided the following interpretive guidance, “An effective immunization program involves collaborating with the medical director to develop resident care policies for immunization(s) that reflect current standards of practice and that include: 

  • Physician approved policies for orders of influenza and pneumococcal vaccines (administration must be based on an assessment of each resident for possible medical contraindications – see F711, for physician orders for vaccinations); 

  • Review of the resident’s record of vaccination and immunization status, including assessment for potential medical contraindications; 

  • How pertinent information and education will be provided to residents or their representatives. The facility may wish to use educational resources such as those provided by the U. S. Centers for Disease Control and Prevention (CDC); and 

  • The vaccination schedule including mechanisms for recording and monitoring for administration of both influenza and pneumococcal vaccines in accordance with national recommendations.

The CDC indicates that administering the influenza vaccine when it becomes available each season, rather than date specific, (i.e., “October 1”) is most effective. Facilities should administer the influenza vaccine when it becomes available to the facility. Residents admitted late in the influenza season (typically February or March) should be offered the influenza vaccine as late season outbreaks do occur. If a resident was admitted outside the influenza season, the facility is not expected to offer the influenza vaccine to the resident, but it may, at its discretion.”

The CDC is recommending that vaccinations against influenza should be administered to healthcare workers by the end of October 2017. Seasonal influenza vaccination resources for health professionals can be found on the CDC website.

The Adult Foster Care (AFC) Council met on August 9. Topics discussed included a scheduled plan to continue reviewing and updating the AFC licensing rules. TRACS User Recertification Notification

On August 23, HUD issued RHIIP Listserv 391 TRACS User Recertification Notification. This Listserv introduces a mandatory requirement to certify TRACS Users and TRACS Coordinators.

The requirement to ensure that TRACS Users and TRACS Coordinators actually need access to resident data located in the TRACS database is mandated by the Federal Information Security Management Act (FISMA) and Office of Management and Budget (OMB).

Starting September 1, TRACS Coordinators will be required to certify all TRACS Users who continue to need access to TRACS data. It is mandatory that Coordinators certify all TRACS Users no later than December 31, 2017. Users who are not certified by December 31, 2017 will no longer have access to TRACS. Coordinators may review HUD's Mandatory TRACS User Certification Instructions available on HUD's website.

MSHDA - Temporary Emergency Housing

In accordance with Revenue Procedures 2014-49 and 2014-50, the Michigan State Housing Development Authority (MSHDA) is permitting all owners of low-income housing tax credit (“LIHTC”) and MSHDA Bond properties within the State of Michigan to provide temporary emergency housing to displaced individuals affected by a Major Disaster. Information about the major disaster declarations can be accessed at

  • Hurricane Harvey received a Major Disaster Declaration on August 25, 2017, with a FEMA incident period beginning on August 23, 2017.
  • Hurricane Irma received a Major Disaster Declaration on September 10, 2017 with a FEMA incidence period beginning on September 04, 2017.

The MEPAP course prepares the activity professional for all aspects of their role. The nationally approved course curriculum is designed to enhance the skills and knowledge required to properly facilitate the activity needs for all residents in aging services settings. The 18-day course offers 180 hours of instructor-led training and 180 hours of supervised practicum assignments. After successfully completing the course, individuals will be prepared for the National Certification Council for Activity Professionals (NCCAP) exam and certification application process.
