 PACE Programs

Programs for All-inclusive Care for the Elderly (PACE) are designed to specifically meet the social, medical, and supportive needs of the very frail senior. The program provides comprehensive acute, primary, and long term supports and services to those who are enrolled in both Medicare and Medicaid programs.  A team of professionals provides coordinated care under the direction of a physician, allowing very frail seniors to remain at home.  Program participants must be at least age 55, live in an area serviced by a PACE organization, require a significant level of care similar to those who need nursing home,  and must be able to live safely in the community.  The PACE program becomes the sole source of services for Medicare and Medicaid beneficiaries.  

There are currently six programs in Michigan, although there are several more in development.  PACE programs are known as the most effective home and community based program in meeting the needs of the frail senior and delaying or avoiding nursing home placement.
