APEX

About Patient Experience

Facing a rapid and aggressive rebuilding of America’s health care and long term care systems, LeadingAge Michigan members focus on several concrete next steps to effect positive change for Michigan’s frail seniors.  One of these initiatives is called APEX (About Patient Experience), an exclusive collaboration that promotes ongoing collection of critical data metrics, evidence based practice, and shared experience to drive superior performance.

APEX uses a certified patient safety organization as its foundation to identify adverse events and capture improvement opportunities. In addition, APEX fosters sharing of best practices and information across provider groups regionally in its Safe Tables.  APEX organizations have embarked on two significant improvement projects for 2015: Reducing Falls and Improving Staff Satisfaction. Please contact Deanna@LeadingAgeMI.org for more information on these two initiatives.  We will be holding kick off meetings in mid-January.

The successful provider in this new health world must deliver services born from evidence, address and streamline care transitions, and foster partner and collaboration in an environment protected by a certified patient safety organization.
